A $30.00 non-refundable application fee
A copy of your high school transcript (copies of diplomas are not acceptable)
A 1-2 page essay explaining why you are interested in the Sunrise School of Dental Assisting, as well the assets you believe you will bring to the dental assisting profession.

A $100 deposit is required to hold your spot in the class. This deposit is applied toward tuition, unless you choose not to attend the program, in which case the fee is non-refundable. Paying the $100 deposit at the time you submit your application can automatically reserve your seat in the class.
